ii) Employer’s bank account information (front cover of bank statement only)
iii) The Business Registration Number (BRN) details used when opening the bank account
iv) SSM registration certificate such as Form 9 (CA 1965) or Section 17 (CA 2016)
v) Declaration PSU50 Form
vi) Supporting documents such as management account and sales report. These documents need to be verified by management**
